TRICARE IOP and PHP in Texas: TriWest Rules and 2026 Costs

Yes. TRICARE covers IOP and PHP for service members and families, and in Texas the approval runs through TriWest. This guide is for Texas military families checking approval steps, 2026 costs and Open Season.

TRICARE IOP and PHP coverage applies to active-duty service members, spouses and children when the care is medically necessary, and in Texas, TriWest Healthcare Alliance, the TRICARE West Region contractor, lists both programs among the services it must approve in advance. Mindful Health, in network with TRICARE, describes its schedule on its intensive outpatient program in Texas page. Texas joined the TRICARE West Region on 1 January 2025.

Does TRICARE cover IOP and PHP?

Yes. TRICARE’s own pages confirm it covers intensive outpatient programs (IOP) and partial hospitalization programs (PHP). Both cover mental health and substance use care. Both pages were last updated on 1 October 2026.

TRICARE says you can qualify for IOP if you need stabilisation or relapse prevention and do not need a full-time hospital stay. You also need more care than regular outpatient visits give you. That includes people stepping down from a hospital, residential treatment or PHP. For PHP, you need a serious impairment in daily functioning and cannot manage with lower-level outpatient care.

The TRICARE Policy Manual describes IOP as a day or evening program of 6 to 9 or more hours a week. Each treatment day lasts at least 2 hours. Mindful Health’s IOP runs about 3 hours a day, 3 to 5 days a week. That fits the definition.

Medical necessity decides approval. Not sure a family member is at that level? Our checklist of signs you need an IOP walks through five of them.

Who runs TRICARE mental health care in Texas now?

TriWest Healthcare Alliance runs TRICARE in Texas. On 1 January 2025, new regional contracts moved six states, Texas among them, from the East Region to the TRICARE West Region. The contractor for those states changed from Humana Military to TriWest. TRICARE Communications put the number of people affected at about 1.1 million.

For Texas families, TriWest mental health rules now govern referrals and approvals. That includes IOP and PHP. Guides written before 2025 often still name Humana Military. If a page tells you to call Humana about care in Texas, it is out of date.

TriWest also sets the paperwork. Its February 2026 mental health checklist says IOP and PHP need a referral from your primary care manager (PCM) or a pre-authorization. Requests go on TriWest’s referral and authorization form, with notes that show why the care is needed.

Do you need a referral or authorization for TRICARE IOP?

Plan on a TriWest prior authorization for both IOP and PHP, whatever your plan. TriWest’s prior authorization list for TRICARE West, last revised on 14 March 2025, names both programs and applies to everyone in the programs TriWest runs.

Some national pages say something else. TRICARE’s PHP page says no pre-authorization is needed. The TRICARE Policy Manual says IOP needs a PCM referral but no preauthorization. In Texas, TriWest approves the claims, so follow its list.

Referrals then depend on who you are. TRICARE’s September 2026 guide to mental healthcare sets out the paths:

  1. Active-duty service members use a military hospital or clinic when possible. Civilian care needs a referral and pre-authorization through their PCM.
  2. Other TRICARE Prime members need no referral for network office visits. PHP needs a referral and pre-authorization.
  3. TRICARE Select members need no referral for most mental health visits.

Our position: ask the program to send the TriWest request before the first session. Thomas Turnbaugh, LCSW-C, a mental health specialist at the Defense Health Agency, said it plainly: “Getting mental healthcare through TRICARE doesn’t have to be complicated.”

TriWest’s list also covers transcranial magnetic stimulation. Mindful Health’s TMS for treatment-resistant depression follows the same approval step.

What does TRICARE IOP or PHP cost in 2026?

For most military families using network care, very little. Active-duty service members pay nothing out of pocket. TRICARE’s 2026 cost table lists outpatient mental health care and partial hospitalization for active-duty family members at $0 in network under TRICARE Prime.

TRICARE Select costs more. For Group A families, whose sponsor joined before 1 January 2018, the same row shows a $39 network copayment. Select also has a yearly deductible. For families of E-5 and above, it is $150 a person or $300 a family in Group A. In Group B, it is $198 or $397.

The yearly catastrophic cap limits the total. For active-duty families on Select, it is $1,000 per family in Group A and $1,324 in Group B. Once you reach it, covered care costs nothing more that year.

Retirees and their families pay different rates, so check your figures at the TRICARE costs page before treatment starts.

That gap shapes the Prime vs Select choice. If a family member is likely to need IOP or PHP next year, Prime is the cheaper choice for active-duty families. Network outpatient mental health care and PHP cost $0 under Prime. Select in Group A charges a $39 network copayment plus the deductible.

Select suits families who want to pick providers without going through a PCM. That freedom counts for less with IOP and PHP, because TriWest’s approval applies on both plans.

Our position: for active-duty families with a child or spouse already in treatment, Prime gives the lowest bill. Families who value choosing specialists directly can accept Select’s copayments and cap.

When is TRICARE Open Season 2026?

TRICARE Open Season runs from 9 November to 8 December 2026, and any change takes effect on 1 January 2027. During that window, you can enrol in TRICARE Prime or TRICARE Select, or switch between them. If you want to keep your current plan, you do nothing.

Open Season does not apply to everyone. TRICARE’s Open Season page excludes active-duty service members, TRICARE For Life, TRICARE Reserve Select, TRICARE Retired Reserve and TRICARE Young Adult.

Outside Open Season, you need a qualifying life event (QLE), such as a move, a birth or a change in the sponsor’s status. A QLE opens a 90-day window for enrolment changes. One family member’s QLE opens that window for every eligible member of the family.

If a family member is already in IOP or PHP, keep the same plan until the program ends. Switch only if it clearly lowers your cost. Changing plans on 1 January means checking that the authorization still stands.

What is Operation Resilience?

Operation Resilience is Mindful Health’s four-week partial hospitalization program for active-duty service members. It runs 20 treatment days, Monday to Friday, 8.30 am to 3.30 pm, in small groups of 12 to 15 service members.

The Operation Resilience program treats trauma, operational stress, anxiety, depression, moral injury, sleep problems and difficulty adjusting. Each week has a focus: Foundation and Awareness, Skills and Resilience, Integration and Growth, and Prepare and Perform. Outdoor, nature-based outings run every week, and participants have psychiatric and nurse practitioner support.

Mindful Health’s team can verify TRICARE benefits, check whether authorization is needed and recommend the right level of care before a group starts. Because it is a PHP, TriWest’s prior authorization applies, and active-duty members need their PCM referral in place first. The program page lists no single clinic, so ask which location hosts the next group.

What can military spouses and children use?

TRICARE family members can use Mindful Health’s psychiatry, therapy, IOP and PHP. Operation Resilience is for active-duty members only, so spouses, children and other dependents use the standard programs.

Mindful Health’s partial hospitalization program meets 5 hours a day, 5 days a week, for 25 hours. Its IOP runs about 3 hours a day, 3 to 5 days a week, in person or virtually.

Spouses who are pregnant or have just given birth can use Mindful Maternal’s perinatal IOP. It runs in person and virtually, for people who need more than an hour of therapy a week.

For teens, Mindful Health’s adolescent IOP and PHP serve ages 12 to 18 who are still in high school. Teens live at home throughout. IOP leaves more of the school day free than PHP. Ask about school coordination before enrolling, especially around a permanent change of station move.

TRICARE covers the care; TriWest’s approval sets the start date

TRICARE covers IOP and PHP for every family member. In Texas, TriWest’s approval decides when treatment can start. Active-duty members, and Prime family members going into PHP, also need a PCM referral. Get the request sent before the first session, check your 2026 costs, and use Open Season to choose the plan that fits 2027.

Request an assessment and TRICARE benefits check with Mindful Health or call (888) 877-6395. In a crisis, call 911, or dial 988 and press 1 for the Military and Veterans Crisis Line.

FAQs About Tricare IOP

Does TRICARE Select need a referral for IOP?

No referral for most mental health visits, but plan on authorization. TriWest’s prior authorization list, revised on 14 March 2025, names IOP and PHP for all TRICARE beneficiaries it serves, including Texas. The program usually sends the request on TriWest’s referral and authorization form, with notes showing medical necessity.

Can I switch from TRICARE Select to Prime outside Open Season?

Only after a qualifying life event, such as a move, a birth or a change in the sponsor’s status. A QLE opens a 90-day window, and it applies to every eligible family member. Otherwise, wait for TRICARE Open Season, from 9 November to 8 December 2026, with changes starting on 1 January 2027.

Does TriWest cover mental health care in Texas?

Yes. TriWest Healthcare Alliance has run TRICARE in Texas since 1 January 2025, including psychiatry, therapy, IOP and PHP. Mindful Health is in network with TRICARE for eligible service members and military dependents. Its team checks whether approval is needed before care starts.

Does TRICARE cover virtual IOP?

Yes, with the same approval step. TriWest’s June 2025 behavioural health guide states that virtual IOPs also need prior authorization for all beneficiaries. Mindful Health runs its IOP in person or virtually for patients in Texas, about 3 hours a day, 3 to 5 days a week.
