Your first appointment at Mindful Health is called an intake appointment, and it’s an important first step in creating your personalized care plan.
For in-person appointments:
When you arrive, you’ll be greeted by our front desk team and asked to complete any remaining intake paperwork. We’ll verify your insurance and collect basic information to ensure your chart is ready for your provider. Once checked in, you’ll meet privately with your clinician or psychiatric provider in a comfortable, welcoming space. During this session, your provider will take time to understand your history, current symptoms, goals for treatment, and any previous care you’ve received. Together, you’ll discuss next steps — whether that includes therapy, medication management, or a specialized program like PHP, IOP, or holistic wellness services.
Our goal during your intake is for you to feel heard, supported, and confident in your plan moving forward.
For virtual appointments:
You’ll receive a secure video link before your scheduled time. We recommend logging in 10 minutes early to test your connection and ensure privacy. The intake follows the same structure as an in-person appointment — your provider will review your history, discuss your goals, and outline your individualized plan for care.
How to prepare:
To make the most of your first appointment, please bring a photo ID, insurance card, a list of current medications, and any recent medical or mental health records that may help your provider better understand your history. If you’re attending virtually, have these items accessible for reference.
